Job summary

Angels Care Home Health in Lubbock, TX is seeking a dedicated Registered Nurse (RN) Case Manager to coordinate all aspects of the patient's home care. The RN will perform admission visits, assess eligibility for home care services, and develop the plan of care in collaboration with medical teams.

Requirements include a state RN license, nursing degree, a valid driver’s license, CPR certification, Hepatitis consent/declination, and at least one year of RN experience (two preferred).

Qualifications

• Current state license as a Registered Nurse required.
• Proof of CPR and Hepatitis consent/declination.
• One year of RN experience preferred (two years ideal).
• Valid driver’s license and reliable transportation required.

Responsibilities

• Coordinate all aspects of the patient’s home care as RN Case Manager.
