Job summary

Ambercare Hospice in New Mexico seeks a Registered Nurse Case Manager to provide compassionate, patient- and family-centered clinical care. You will manage caseloads, coordinate visits, and guide care planning to ensure comfort, dignity, and quality of life for patients at home or in the community.

Qualifications

  • Graduate from an accredited registered nursing program
  • Licensed RN in the state of practice
  • CPR and First Aid Certification
  • Knowledge of quality assessment and assurance procedures
  • Valid driver's license and proof of insurance

Responsibilities

  • Assume primary responsibility for patient and family caseload, including assessment, eligibility, admission processing, planning, implementation, and evaluation phases of the nursing process
  • Obtain data from patient, family and environment to assess needs and engage the inter-disciplinary team
  • Initiate communication with attending physician and other agencies to coordinate optimal care
  • Provide regular visits to reassess patient needs and update plans of care
  • Perform clinical pronouncement of death and provide family support
  • Maintain accurate patient records, charts, progress notes, and care plans timely
  • Supervise and document delivery of care by Home Health Aide as required by guidelines

What You'll Do
  • Assume primary responsibility for patient and family caseload, including the assessment, determining eligibility, admission processing, planning, implementation, and evaluation phases of the nursing process
  • Obtain data and assess necessary information from patient physical, psychological, social, and spiritual factors that may impact the patient and family's needs and initiate intervention by other members of the interdisciplinary team
  • Initiate communication with the attending physician, interdisciplinary team, and other agencies for the purpose of coordinating optimal care
  • Provide regular visits to re-assess the patient's needs and update the plan of care
  • Perform clinical pronouncement of death and provide support to the family
  • Maintain accurate patient records, charts, progress notes, reports and prepare care plans timely
  • Supervise and document delivery of care by the Home Health Aide as required by Federal, State, and Company guidelines
Qualifications
  • Graduate from an accredited registered nursing program
  • Licensed as a registered nurse in the state of practice
  • Must possess current CPR, First Aid Certification
  • Must be knowledgeable of quality assessment and assurance procedures
  • Valid driver's license and proof of insurance is required
