Job summary

Mentor Community Services, part of the Sevita family, seeks a Registered Nurse Traveling to join a mission-driven team providing community-based health care. The RN will travel between multiple locations, conducting assessments, coordinating plans, and ensuring medical information is accurately communicated with physicians and centers.

You will help implement health care plans, oversee staff training, and ensure orders are followed as prescribed while earning $47/hr for 32–40 hours per week.

Qualifications

• RN licensure and graduation from an accredited RN program.
• Minimum one year of experience with the population served.
• Strong assessment abilities and critical thinking in clinical settings.
• Effective communication with staff, medical professionals, patients, and families.
• Valid driver’s license, car registration, and auto insurance.
• Current CPR/First Aid certification.

Responsibilities

• Conduct assessments and implement health care plans for individuals.
• Communicate with physicians and external medical centers to ensure essential medical information.
• Provide medical oversight and staff training for service implementation.
• Contribute input on health services in each individual’s service plan.
• Monitor physician orders and ensure they are carried out as prescribed.
• Travel between multiple locations to provide support and be accessible to staff.

Registered Nurse Traveling

$47/hr

32-40 hrs/week

The heart of what we do is support and serve individuals with intellectual and developmental disabilities, brain injury, substance abuse disorder, and/or living with emotional or behavioral disabilities.

• Conduct assessments, implement health care plans, and respond to changes of condition

• Communicate with the physician and external medical centers to ensure consistency of essential medical information

• Provide technical assistance, medical oversight, and training of staff to ensure the effective implementation of all related services

• Provide input regarding health care services to be included in each individual’s service plan

• Monitor and ensure physicians orders are documented and carried out as prescribed

• Travel between multiple locations to provide support and be accessible to staff at all locations

Qualifications:

• Graduate of an accredited RN program

• One year of experience working with the population we serve

• Current state RN License

• Current driver's license, car registration, and auto insurance

• Current CPR/First Aid certification

• Excellent assessment skills with the ability to exercise critical thinking in the clinical environment.

• Effective communication skills - ability to communicate clearly with team members, medical professionals, those we serve, and their families
