Angels Care Home Health is seeking a Registered Nurse (RN) Case Manager in Texas. You will coordinate home care, interact with multidisciplinary teams, perform admission visits, and develop care plans tailored to patient needs.

Qualified candidates have an RN license, 1 year of RN experience (2 preferred), CPR, and a valid driver's license. We offer a competitive package with benefits and opportunities for advancement.

• Graduate of an accredited diploma, Associate or Baccalaureate School of Nursing.
• Current state license as a Registered Nurse and valid driver's license.
• One-year RN experience; two preferred.
• CPR certification and hepatitis consent/declination.
• Reliable transportation with valid auto liability insurance.

About the position:

The Registered Nurse (RN) Case Manager coordinates all aspects of the patient’s home care. You will interact with other medical disciplines as needed & provide professional care to the patient. You will also perform the admission visit to the patient in the home & determine the patient’s eligibility for home care services & develop the plan of care to be followed.

Qualifications:

• Graduate of an accredited diploma, Associate or Baccalaureate School of Nursing.
• Current state license as a Registered Nurse, Current state Driver’s License.
• One-year experience as a Registered Nurse, two preferred.
• Proof of current CPR and Hepatitis consent/declination.
• Reliable transportation with valid and current auto liability insurance.
