Job summary

Angels Care Home Health is seeking an Registered Nurse (RN) Case Manager to coordinate all aspects of patient home care in Knoxville, IA. You will interact with multiple medical disciplines and perform admission visits to determine eligibility for home care services and develop the plan of care to be followed.

Qualified candidates hold a nursing diploma/associate/BSN, current RN license, CPR, and a valid driver’s license.

Qualifications

• Graduate of an accredited nursing program (Diploma, Associate, or BSN).
• Current State RN license.
• CPR certification required.
• Reliable transportation to patient homes and valid driver's license.

Responsibilities

• Coordinate all aspects of patient home care as RN Case Manager.
• Perform admission visits and determine eligibility for home care services.
• Develop and monitor the plan of care in collaboration with the healthcare team.
• Ensure high-quality patient care and documentation in the EMR system.
