Job Title: Residential RN

SUMMARY
The Registered Nurse (RN) works as a member of the Program Support Team, providing medical oversight of residents through direct observation and consultation. The RN assists Program Managers, the Shared Living Program Director, and Shared Living Contractors with supervision of the health and safety of Community Living and Shared Living individuals served by Life Unlimited.
BASIC FUNCTIONS AND RESPONSIBILITIES
- Ensure the health of all individuals.
- Provide a minimum of 1.25 hours per month per resident, including face-to-face visits of at least 15 minutes in the home. Visits at day programs should include medication review, home environment assessment, and collaboration with in-home staff.
- Monitor residents' health and safety monthly.
- Review medical documentation, physician orders, and medication administration records (MAR) regularly.
- Assess each individual monthly to evaluate health concerns, including face-to-face visits, interviews with support staff, and review of medical documentation.
- Review compliance with physician orders, treatments, procedures, labs, appointments, diets, medications, outcomes, and adaptive equipment.
- Obtain initial health history and perform physical examinations within four weeks of admission.
- Consult on complex medical/mental cases, keeping the community informed of health and safety needs.
- Inspect medication procedures periodically, including storage, destruction, and documentation.
- Maintain documentation of time and activities for audits.
- Review annual ISP and addendums for accuracy of health information and supplies.
- Attend monthly health team meetings in person.
- Oversee implementation of health policies, recommending revisions as needed.
- Communicate timely and effectively with all stakeholders.
- Coordinate with the VP of Health Services for resident health care.
- Participate in staff and house meetings to address corrective actions.
- Provide health education to staff and contractors as necessary.
- Maintain open communication with individuals, families, providers, and agencies.
- Collaborate with Quality Assurance RN’s to implement statewide health initiatives.
- Use computers or mobile devices efficiently for data management.
- Train agency personnel on health matters.
- Become certified and conduct Level 1 Medication Aide training monthly.
- Maintain certification and conduct updates for certified staff.
- Delegate and supervise unlicensed personnel performing nursing duties, documenting activities according to law.
- Inspect medication passing techniques and address errors.
- Perform other duties as assigned by the Executive Director or mandated by health authorities.
Physical Expectations
Extended sitting, walking, standing, bending, kneeling, stooping, climbing stairs, lifting up to 50 pounds, and driving may be required. Use of computers and calculators for extended periods is expected.
Working Conditions
Office/home environment with potential exposure to infectious diseases and bloodborne pathogens.
QUALIFICATIONS
- Active license in good standing with the Missouri State Board of Nursing.
- Knowledge of physical needs and medical implications of individuals with developmental disabilities.
- Level 1 Medication Administration trainer certification (must be acquired if not already held).
- Minimum age of 21 years.
- Class E driver’s license with minimum auto insurance coverage.
