Job summary

Home Services Unlimited is seeking a compassionate RN Case Manager to provide skilled nursing care in patients' homes across the Indianapolis area with autonomy in caseload management.

Full-time and part-time opportunities are available; benefits for 30+ hours per week. Join a supportive clinical team at a trusted local agency with strong patient-family relationships.

Qualifications

• Active Indiana RN license
• ASN or BSN required or preferred
• Home health experience preferred
• Experience using EHR systems preferred
• Strong clinical assessment, communication, and organizational skills
• Ability to work independently and manage a patient caseload
• Valid driver's license and reliable transportation

Responsibilities

• Develop, implement, and evaluate individualized care plans
• Provide skilled nursing care and treatments in the home setting
• Educate patients and caregivers on disease management, medications, and care plans
• Coordinate care with physicians, therapists, and other healthcare professionals
• Supervise LPNs and support staff as needed
• Complete accurate and timely electronic documentation
• Ensure compliance with agency policies, Medicare regulations, and clinical standards
• Participate in care conferences, team meetings, and quality improvement initiatives (QAPI)

Responsibilities

As an RN Case Manager, you will:

Develop, implement, and evaluate individualized care plans

Job Types: Full-time

Compensation: $50-$110 per visit or Salary for highly experienced candidates.

Work Location: Field-based, providing care in patients' homes throughout the Indianapolis area

Schedule: Flexible scheduling based on patient needs and caseload
