Job summary

Bayonet Point Health Center by Harborview in Florida is seeking a dedicated Registered Nurse to provide high-quality nursing care in a skilled nursing/long-term care setting. You will supervise CNAs, collaborate with the IDT to create personalized care plans, and ensure timely assessments, documentation, and rounds.

The role emphasizes patient-centered care within facility policies, with ongoing administrative duties and coordination with physicians to address resident needs.

Qualifications

• Active RN license in good standing in the state of practice.
• Experience in a skilled nursing or long-term care setting preferred.

Responsibilities

• Supervise and support CNAs under the direction of the Director of Nursing.
• Collaborate with the interdisciplinary team (IDT) to assess, plan, implement, and evaluate individualized care plans.
• Ensure initial baseline and periodic comprehensive assessments are completed on time.
• Prepare for and facilitate physician rounds by flagging concerns and preparing documentation for review.
• Initiate consultations or referrals as appropriate based on resident needs.
• Perform administrative tasks including medical documentation, evaluations, and reports.
• Deliver nursing care within scope of practice and aligned with facility policy and clinical standards.
• Monitor residents for changes in condition and notify providers as needed.
• Perform other duties as assigned by the Director of Nursing.
