Job summary

National Recruiters is seeking Registered Nurses for immediate opportunities across New Jersey. In-Home Assessments involve traveling to patients' residences to perform comprehensive health evaluations, with flexible scheduling and field-based work that fits independent, patient-centered care.

As an In-Home Assessment RN, you will review medical histories, reconcile medications, record vital signs, and document findings in EMR.

In-Home Health Assessments | Immediate Openings | Flexible Schedule

National Recruiters is currently seeking Registered Nurses for immediate opportunities throughout New Jersey.

We have an immediate need for experienced RNs who are looking for flexible, field-based work and are comfortable traveling to patients' homes to complete comprehensive health assessments.

With multiple territories currently available, this is a great opportunity for nurses who enjoy working independently, interacting directly with patients, and using their clinical skills outside of a traditional hospital or office setting

The Opportunity

As an In-Home Assessment RN, you will visit patients in their homes and complete a comprehensive health assessment designed to identify current healthcare needs and provide the care team with an accurate picture of the patient's overall health.

Your responsibilities may include:

• Reviewing medical history and current health conditions
• Completing medication reconciliation
• Performing a review of systems
• Obtaining and documenting vital signs
• Conducting a comprehensive physical assessment
• Identifying and documenting current and historical diagnoses
• Performing appropriate point-of-care testing when required
• Using your clinical judgment to identify potential health concerns
• Accurately documenting assessment findings in an electronic medical record
• Communicating professionally and compassionately with patients

Most visits take approximately 35-45 minutes to complete.

Compensation

Your per-visit rate is based on your monthly commitment:

Monthly Commitment

Pay

50-60 hours/month $50 per visit

70-80 hours/month $55 per visit

80+ hours/month $60 per visit

Minimum commitment: 50 hours per month

The ability to take on additional hours may be available based on patient volume and geographic coverage.

We are currently recruiting RNs who can provide coverage in at least 3 New Jersey counties.

• Ocean
• Salem
• Somerset
• Sussex

What We're Looking For

• Active, unrestricted New Jersey RN license
• Minimum of 3 years of clinical experience
• Strong patient assessment and clinical judgment skills
• Ability to work independently in patients' homes
• Excellent communication and interpersonal skills
• Strong documentation and EMR skills
• Professional, dependable, and organized
• Comfortable traveling within your assigned territory
• Availability to commit to at least 50 hours per month

Experience with primary care, adult, geriatric, home health, community health, or similar patient populations is preferred.
