Job summary

Angels Care Home Health in Nebraska seeks a Registered Nurse (RN) Case Manager to coordinate all aspects of patient home care. You will interact with other medical disciplines and provide professional in-home care, including admission visits to determine eligibility and develop the plan of care.

The position requires an accredited nursing degree, active RN license, a valid driver’s license, CPR and Hepatitis consent, and at least 1 year of RN experience (2 years preferred).

Qualifications

• Graduate of an accredited nursing program (diploma/ADN/BSN) with RN licensure.
• Active RN license in the appropriate state.
• Valid driver's license and reliable transportation.
• CPR certification and Hepatitis consent/declination.
• 1 year RN experience; 2 years preferred.

Responsibilities

• Coordinate all aspects of the patient’s home care and liaise with medical teams.
• Perform admission visit to home and assess eligibility for home care services.
• Develop and implement the plan of care for home health services.
