Registered Nurse (RN) – Community-Based Care

Provide compassionate, person-centered nursing care to individuals of all ages and abilities in the community. Perform comprehensive assessments, develop and implement individualized care plans, and administer medications and treatments. Maintain accurate clinical documentation and ensure patient safety while meeting regulatory requirements.

Collaborate with an interdisciplinary care team, educate individuals and families, and support positive health outcomes through effective communication and care coordination.
