Azza HealthCare Agency in Kentucky is seeking a dedicated registered nurse to provide compassionate home- and community-based care. You will assess health needs, develop and implement individualized plans, administer medications, and coordinate with an interdisciplinary team to ensure optimal patient outcomes.

You will educate patients and families, document care accurately, and contribute to a collaborative care environment that supports patient independence and well-being in the community

Formación

• Active Registered Nurse licensure.
