Kindred Hospital Denver and Acute Rehabilitation Unit (ARU)

68-bed private room hospital located east of St. Joseph’s Hospital and Presbyterian St. Luke’s Medical Center. 68 long‑term acute care hospital (LTACH) beds offering in‑depth care for an extended recovery period. 13‑bed ARU designed for patients with acute injury, impairment or illness. 24‑hour clinical care seven days a week to start the journey to wellness. Care touches individuals from the Front Range, western slopes of Colorado and throughout the country.

Job Summary

Provides planning and delivery of direct and indirect patient care through the nursing process of Assessment, Planning, Intervention, and Evaluation. Develops nursing care plans in coordination with patient, family and interdisciplinary staff as necessary. Communicates changes in patient’s clinical condition with Physicians, Nursing Supervisor/Manager, and co‑workers as appropriate. Participates in discharge planning process.

Essential Functions
  • Maintains the standard of nursing care and implements policies and procedures of the hospital and nursing department.
  • Directs, supervises, and evaluates nursing care provided to patients.
  • Assigns or delegates tasks based on needs, potential for harm, task complexity, and scope of practice.
  • Assigns nursing care team members according to patient needs and team member capabilities.
  • Documents patient admission assessment, care plans, and other pertinent information in the medical record per nursing standards and policies.
  • Performs assessment on all patients at admission and reassessments as per policy. Makes referrals to other disciplines based on assessment.
  • Develops and updates nursing care plan for assigned patient, ensuring coordination with patient, family, and team.
  • Assesses and reassesses pain; applies appropriate pain management techniques and educates patient and family.
  • Revises plan of care as indicated by patient’s response and evaluates effectiveness daily.
  • Performs patient care responsibilities considering age‑specific standard of care.
  • Receives physician orders, ensures accurate transcription, and documents completion.
  • Administers medication using the five rights of medication administration to reduce errors.
  • Formulates teaching plan based on learning needs and evaluates effectiveness; includes family as appropriate.
  • Assists physicians with examinations, treatments, and special procedures within scope of practice.
  • Performs treatments and provides services at level of licensure.
  • Treats patients and families with respect and dignity; addresses psychosocial, cultural, ethnic, and religious/spiritual needs.
  • Functions as liaison between administration, patients, physicians, and other providers.
  • Interacts professionally with patient/family and involves them in care plan formation.
  • Interprets patient data to identify age‑specific needs and provides required care.
  • Performs all patient care aspects in a patient‑safety environment, reducing likelihood of medical errors.
  • Initiates or assists with emergency measures for sudden adverse developments in patient condition.
  • Answers telephone, paging system, patient call lights, anticipates needs, and responds during rounds.
  • Consults other departments as appropriate to provide interdisciplinary approach to patient needs.
  • Provides end‑of‑shift report to oncoming nurse, counts narcotics, documents completion, and signs off physician orders.
  • Communicates appropriately and clearly to management, co‑workers, and physicians.
  • Identifies and addresses psychosocial needs of patients and family; coordinates with Social Service/Discharge Planner.
  • Participates in orientation, instruction/training of new personnel.
  • Manages and operates equipment safely and correctly.

Pay Range

$41.84 – $53.06/hr

Qualifications
Education
