Lake Pointe Rehabilitation & Nursing Center in Conneaut, Ohio is seeking a dedicated Registered Nurse to join our rehabilitation team. You will provide comprehensive nursing care to short-term residents and collaborate with an interdisciplinary team to help residents reach their rehabilitation goals.

The role requires an Ohio RN license with a BSN preferred, at least 1 year in rehabilitation nursing, strong communication, empathy, and the ability to manage priorities in a fast-paced setting.

Qualifications

• Current Registered Nurse license in the state of Ohio.
• Bachelor’s degree in Nursing (or related field)
• Minimum of 1 year of experience in rehabilitation nursing (or related field)
• Strong communication and interpersonal skills
• Ability to work in a fast-paced environment with multiple priorities
• Compassion and empathy for patients and their families

Responsibilities

• Provide comprehensive nursing care to our short-term rehabilitation residents in collaboration with our interdisciplinary team
• Develop and implement individualized care plans tailored to each resident's unique needs and goals
• Conduct patient assessments, develop and implement care plans, and evaluate patient progress
• Collaborate with rehabilitation therapists, physicians, and other healthcare professionals to ensure seamless care coordination
• Educate patients and their families on care plans, medications, and treatment options
• Participate in quality improvement initiatives to ensure optimal patient outcomes
• Maintain accurate and thorough patient records
