Job summary

Angels Care Home Health is seeking a Registered Nurse Case Manager to coordinate patient home care and participate in admission visits in Missouri. You will determine home health eligibility, develop care plans, and work with a multi-disciplinary team to deliver quality nursing services in patients’ homes.

The role requires an active Missouri RN license, a valid driver's license, and at least one year of RN experience.

Qualifications

• Graduate of an accredited nursing program (diploma/ADN/BSN).
• Current RN license in the state (Missouri).
• Valid driver's license and reliable transportation.

Responsibilities

• Coordinate all aspects of the patient’s home care as an RN Case Manager.
• Perform admission visits and determine eligibility for home care services.
• Develop and monitor the plan of care with interdisciplinary team.
• Provide professional, compassionate nursing care in patient homes.

You will interact with other medical disciplines as needed & provide professional care to the patient.
