**Job Description:**
We are seeking a dedicated and experienced Registered Nurse (RN) to join our team as a Veteran and Medicaid Client Assessor. The RN will be responsible for conducting comprehensive assessments of veteran/medicaid clients to determine their individual care needs and create personalized care plans. This role requires excellent clinical skills, a compassionate approach, and a strong understanding of the unique needs of veterans.
**Key Responsibilities:**
1. **Conduct Assessments:**
- Perform thorough initial and ongoing assessments of veteran/medicaid clients in their homes.
- Evaluate physical, emotional, and psychological needs.
- Identify specific care requirements and potential risks.
2. **Develop Care Plans:**
- Create individualized care plans based on assessment findings.
- Collaborate with clients, families, and other healthcare professionals to ensure comprehensive care.
- Adjust care plans as needed to reflect changes in the client's condition.
3. **Monitor Client Progress:**
- Regularly review and update care plans.
- Track and document client progress and any changes in condition.
- Provide feedback and recommendations to the care team.
4. **Coordination and Communication:**
- Coordinate care services with other healthcare providers, community resources, and family members.
- Communicate effectively with clients, families, and the Visiting Angels team.
- Serve as a liaison between clients and external healthcare providers.
5. **Education and Support:**
- Educate clients and families about care plans, medical conditions, and available resources.
- Provide support and guidance to clients and their families throughout the care process.
- Ensure clients understand their care plans and the importance of adherence.
- Ensure all Caregivers on staff have completed the necessary Medicaid training
6. **Compliance and Documentation:**
- Maintain accurate and timely documentation of assessments, care plans, and client interactions.
- Ensure compliance with all regulatory requirements and Visiting Angels policies.
- Participate in audits and quality assurance activities as required.
- Sign off on Medicaid compliance documents
**Qualifications:**
- Valid Registered Nurse (RN) license in the state of Iowa.
- Minimum of 3 years of nursing experience, preferably in home health or geriatric care.
- Experience working with veteran clients is highly desirable.
- Strong clinical assessment and critical thinking skills.
- Excellent communication and interpersonal skills.
- Ability to work independently and as part of a multidisciplinary team.
- Compassionate and patient-centered approach to care.
- Proficient in electronic health records (EHR) and Microsoft Office Suite.
