Key Responsibilities:

Routinely travel to patients' homes to deliver care, educate, conduct evaluations, and offer ongoing support, ensuring personalized and effective treatment.

Demonstrate comfort and professionalism when working in diverse patient home environments, ensuring a compassionate and safe care experience for each individual.

Collects and records relevant patient health data in the course of/pre/post hemodialysis treatment;

Develops and maintains a plan of care that describes interventions to attain expected outcomes;

Implements and records interventions to attain expected outcomes in EMR;

Performs laboratory collection, processing, and shipping procedures and reschedules missed or inadequate laboratory collections;

Perform clerical tasks in addition to patient care (i.e., filing, inventory; management, admission documentation, creating patient charts, etc.);

Maintains a positive and respectful attitude to all patients and colleagues; and

Medication administration and documentation

Requirements:?

Valid RN license

Valid state driver's license

2 years of RN experience required

1 year of hemodialysis required

1 year of Nxstage machine required

BLS certification; and

Ability to lift up to 35 pounds
