Job summary

Qualifications

• Nutrition screening, assessment, monitoring, evaluation and education of patients.
• 1+ year healthcare experience.
• Licensed Dietitian Nutritionist (IDFPR) and Registered Dietitian (CDR).

Responsibilities

• Provide comprehensive nutrition care per policies and procedures.
• Complete nutritional assessments 100% per policy.
• Deliver medical nutrition therapy as appropriate.
• Educate patients on nutrition, digestive health and disease prevention.
• Document assessments and care plans in the EMR.
• Participate in rounds, seminars and team meetings.
