Job summary

BoldAge PACE is seeking a Registered Dietitian to assess nutritional status, plan and implement nutrition interventions, and collaborate with the Interdisciplinary Team to meet the nutritional needs of participants. The role involves counseling, education, and coordination of care within a patient-centered program.

Qualifications

• Bachelor’s degree in food, nutrition, or dietetics.
• Passing the Commission on Dietetic Registration (CDR) exam.
• Minimum of 1 year working with the elderly population.
• Minimum of two years clinical nutrition experience preferred.
• Serve Safe certification preferred.

Responsibilities

• Provide high quality nutritional care as a member of the Interdisciplinary Team (IDT).
• Perform initial nutritional assessments and participate in care planning.
• Provide nutrition counseling and education to participants and caregivers.
• Coordinate with IDT to implement nutrition interventions and monitor outcomes.
• Support 24-hour care delivery planning and IDT communications.
• Incorporate cultural and ethnic preferences into care plans.
• Participate in quality improvement projects and team meetings.
• Maintain confidentiality and documentation standards.

Education

Bachelor’s degree in food, nutrition, or dietetics

CDR exam

1 year elderly population experience

2 years clinical nutrition experience

Serve Safe certification (preferred)
