Job summary

WellSpan Health is seeking a part-time Diabetes Education Specialist to provide group and individual education, coordinate nutrition counseling, and develop care plans for participants. The role supports cross-coverage with the Ephrata Diabetes Education department and may involve occasional travel.

The position requires a nursing degree or related credential, RN or RD/CDCES qualifications, and at least 2 years of relevant experience, with BLS and CDCES timelines. On-site with remote capability.

Qualifications

• Bachelor's Degree in Nursing, Clinical Nutrition or related field.
• 2 years outpatient, inpatient or other relevant experience.
• Licenses required include RN (single state or multi-state), Licensed Dietitian-Nutritionist within 180 days, Registered Dietitian within 90 days, BLS within 90 days, CDCES within 3 years.

Responsibilities

• Provides group education and individual education in Diabetes Education.
• Provides nutritional counseling to clients referred by a provider.
• Collaboratively develops educational goals and plans with participants.
• Triages and monitors glucose levels, medications, CGM patterns and insulin pumps with care team.
• Documents data and evaluation results in participant records.
• Collaborates with the care team and participates in peer review.

48 hours bi-weekly

3 - 8 Hour shifts

Occasional evening or weekends to support community events

Employee would be remote capable but primarily on-site

Position will also have an expectation of supporting our sister site of Ephrata Diabetes Education department for providing cross-coverage and broad service coverage

Occasional travel is expected when needed to sister site or to a community benefit fair

CDCES or certified education preferred but not required, but expected to achieve within 3 years

General Summary

Provides group and individual Diabetes Education. Develops educational goals, learning objectives and a plan for educational content and teaching methods. Develops an individualized follow-up plan of care with each program participant.

Essential Functions

• Provides group education and individual education in Diabetes Education.
• Provides nutritional counseling to clients referred by a provider, as assigned.
• Collaboratively develops educational goals, learning objectives and a plan for educational content and teaching methods with program participants.
• Triages, manages, and monitors blood glucose levels, medications, continuous glucose monitor patterns, and insulin pumps with the program participant and health care team.
• Collects program participant assessment data according to the Diabetes Education Program Key Performance Indicators. Participates in evaluating outcome measures and continuous program improvements for the Diabetic Population. Participates in auditing the program to ensure meeting the program objectives and requirements.
• Documents assessment data, educational plan, educational services provided and evaluation results in each participant’s educational records.
• Utilizes a team approach to provide services and collaborates and communicates with team members when needed.
• Participates in peer review process to evaluate performance of other professional staff.
• Supports clinical team members by providing essential education and collaboration on the care of the program participant.
• Works within their respective scope and practice within the Diabetes Education Program as defined by organizational competencies, policies, procedures, and standard of work.
• Supports community events as identified.

Qualifications

Minimum Education

• Bachelors Degree Degree in Nursing, Clinical Nutrition or related field. Required

Work Experience

• 2 years Outpatient, inpatient or other relevant experience. Required

Licenses

• Licensed Registered Nurse Upon Hire Required or
• Registered Nurse Multi State License Upon Hire Required or
• Licensed Dietitian-Nutritionist within 180 days Required and
• Registered Dietitian / Registered Dietitian Nutritionist within 90 days Required and
• Basic Life Support within 90 days Required and
• Certified Diabetes Care and Education Specialist within 3 years Required
