• Conduct personalized nutrition therapy consultations via telehealth.
• Administer evidence-based clinical protocols and provide individualized nutrition support.
• Monitor patient progress and adjust plans to meet goals and improve outcomes.
• Maintain high patient retention by following clinical guidelines.
• Collaborate with care team members.
• Utilize meal planning tools to support patient compliance.
• Participate in clinical rounds with partner practices.
• Attend quarterly RD group meetings and monthly 1:1s with CNM.
• Support lifestyle changes for patients.
• Build trust with patients and caregivers quickly.
• Show empathy in interactions.
• Document clinical information in electronic medical records.
• Work efficiently in a fast-paced, evolving environment with a positive attitude.
• Follow operational procedures and transition smoothly between patients.

Requirements

• State license in Maryland and/or Tennessee (or willingness to obtain).
• Bachelor's Degree in Nutrition/Dietetics.
• Completion of an Accredited Dietetic Internship.
• Credentialed by the Commission on Dietetic Registration (CDR).
• 1-2 years of post-internship nutrition experience.
• Experience counseling patients virtually or in outpatient settings.
• Expertise in managing chronic conditions like diabetes, hypertension, obesity, etc.
• Experience treating GI conditions such as IBD, IBS, fatty liver, celiac disease, etc.
• Comfort in educating patients with complex conditions including diabetes and kidney disease.
