Job Summary

The RD colleague has the primary responsibility for supporting goals and objectives of the care enhancement program by providing telephonic care and resource utilization for members in an appropriate, efficient, and cost-effective manner while ensuring high quality care. The RD colleague has responsibility for the provision of member care within their scope of practice, coordinating with the Primary Care Provider (PCP), and all other members of the health care team to support appropriate total healthcare management.

RD colleagues are supervised by an Operations Manager. All Sharecare clinicians (including RD colleagues) are required to participate in orientation and to complete pre- and post-tests to review competency during orientation. Yearly competency testing is also required.

Schedule

Monday - Friday, 9:30am - 6pm CT. (non-negotiable)

Due to structured training, new hires cannot miss any days for the first 60 days of employment.

Essential Job Functions

• Deliver dietary services within the RD scope of practice to members with chronic and acute conditions, providing nutritional consults as requested. Assist with coaching calls as needed.
• Follow company policy to transfer calls to an RN or appropriate colleague when needs are outside the RD scope of practice.
• Assess general needs, background, and resources with members and establish mutual goals.
• Intervene based on physician guidelines and diet prescriptions when available.
• Collaborate with other professionals as appropriate.
• Develop and implement nutrition plans supporting physician treatment when applicable.
• Assess nutrient interactions and document findings.
• Identify educational needs and facilitate educational opportunities, including sending educational materials and referring to health plans.
• Discuss evidence-based nutritional guidelines with members.
• Coordinate communication with PCPs, other providers, and team members to ensure continuity of care.
• Gather and document member data, evaluate needs, and coordinate with the PCP and other providers.

Qualifications

This position requires a responsive, committed individual who recognizes the impact of healthcare delivery on clinical and financial outcomes and can facilitate system improvements.

• Current RD Registration is required.
• Minimum 2 years recent RD experience in a clinical setting, preferably with telephonic dietary coaching.
• Bachelor’s Degree required; basic knowledge of physiology and chronic disease management.
• Effective communication skills for engaging with members, colleagues, and providers.
• Proficiency in basic math and data interpretation.
• Computer literacy with Windows systems.
• Ability to educate and promote behavior change in a telephonic environment.
