Job Summary

The RD colleague has the primary responsibility for supporting the goals and objectives of the care enhancement program by providing telephonic care and resource utilization for members in an appropriate, efficient, and cost-effective manner while ensuring high-quality care. The RD colleague is responsible for providing member care within their scope of practice, coordinating with the Primary Care Provider (PCP), and all other members of the healthcare team to support appropriate total healthcare management.

RD colleagues are supervised by an Operations Manager. All Sharecare clinicians (including the RD colleagues) are required to participate in orientation and to complete pre- and post-tests to review competency during orientation. Yearly competency tests are required for all Sharecare clinicians.

Schedule

Monday - Friday, 9:30am - 6pm CT. (non-negotiable)

Due to structured training, new hires cannot miss any days for the first 60 days of employment.

Essential Job Functions
  • Achieve clinical objectives by delivering dietary services within the RD scope of practice to members with chronic and acute conditions, providing nutritional consults upon request, and assisting with coaching calls as needed.
  • Follow company policy to transfer calls to an RN or appropriate colleague when member needs are outside of the RD scope of practice.
  • Collaborate with members to assess needs, background, and resources; establish mutual goals.
  • Intervene based on member understanding of physician guidelines or diet prescriptions.
  • Coordinate with other professionals as appropriate.
  • Apply clinical knowledge to develop effective care plans, assess nutritional needs, and implement plans supporting physician treatment when available.
  • Assess and document member understanding of nutrient interactions and supplement use according to evidence-based guidelines.
  • Identify educational needs and facilitate educational opportunities, such as sending educational materials or referring to health plans.
  • Discuss evidence-based nutritional guidelines with members.
  • Function as part of the multidisciplinary team, facilitating communication with PCPs, providers, health plan representatives, and colleagues.
  • Coordinate care with other RD colleagues for continuity.
  • Gather and evaluate member data, document appropriately, and obtain additional information from PCPs as needed.

Qualifications

This position requires a responsive, committed individual who recognizes the impact of healthcare delivery on clinical and financial outcomes and can facilitate system improvements.

  • Current RD Registration is required.
  • Minimum 2 years recent RD experience in a clinical setting, preferably with dietary coaching in a telephonic environment.
  • Bachelor’s Degree required, with basic knowledge of physiology and chronic diseases.
  • Effective communication skills for interacting with members, colleagues, physicians, and plan representatives.
  • Proficiency in reading, analyzing scientific journals, and presenting information clearly.
  • Mathematical skills for calculations and data interpretation.
  • Ability to solve practical problems and interpret instructions effectively.
  • Basic computer skills and familiarity with Windows OS.
  • Ability to educate and promote behavior change in a telephonic environment.
