Job Description

Job Title: Registered Dietitian (In-home, full-time)

Organization: RD Nutrition Consultants

Location: Bridgeport, CT

Salary: $65,000 per annum

Work Hours: Monday-Friday, Business Hours with occasional evenings

Responsibilities

• 1. Conduct initial in-home consultations for individuals newly diagnosed with diabetes, offering education and support for managing their condition through nutrition.
• 2. Provide practical culinary education, demonstrating the preparation of healthy meals tailored to manage diabetes.
• 3. Distribute fresh fruits and vegetables provided by Walmart to clients as part of their nutritional management, enhancing the program's effectiveness.
• 4. Guide clients in healthy grocery shopping at Walmart, providing practical tips and curated food lists to support their dietary needs.
• 5. Monitor and document patient progress and provide necessary feedback during follow-up sessions.
• 6. Collaborate with healthcare providers and Hartford HealthCare (HHC) to create holistic care plans for clients.
  
  

Qualifications

• 1. Registered Dietitian (RD) certification.
• 2. Proven experience working as a dietitian, ideally in a community or home-based setting. Experience in diabetes management is highly desirable.
• 3. Excellent interpersonal and communication skills, able to build relationships with clients of diverse backgrounds.
• 4. Knowledgeable in creating diet plans and providing culinary education.
• 5. Bilingual skills preferred but not required.

Program Collaboration: This program is in collaboration with Walmart, which provides participants with fresh food and groceries in addition to comprehensive nutrition education, further empowering individuals in managing their diabetes effectively.
