Job summary

AXIA Care Center in New Jersey is seeking a knowledgeable and compassionate Registered Dietitian to join our interdisciplinary team in a full-time role within a long-term care and rehabilitation setting. You will assess residents' nutritional needs and develop individualized care plans.

The ideal candidate will collaborate with Nursing, Dietary Services, physicians and families, document thoroughly, support quality-improvement efforts, and ensure regulatory compliance while promoting residents'

Qualifications

• Current Registered Dietitian.
• Bachelor's degree in nutrition, dietetics, or a related field.
• Compliance with all applicable state credentialing requirements.
• Long-term care, skilled nursing, rehabilitation, or clinical dietetics experience preferred.
• Strong assessment, documentation, organization, and communication skills.
• Ability to work collaboratively as part of an interdisciplinary care team.

Responsibilities

• Complete comprehensive nutritional assessments and timely follow-up evaluations.
• Develop individualized nutrition recommendations and care plans.
• Monitor residents experiencing weight changes, altered intake, wounds, renal concerns, tube feeding, or other nutritional risks.
• Participate in care-plan meetings and collaborate with Nursing, Dietary Services, physicians, rehabilitation staff, residents, and families.
• Review diet orders, supplements, enteral-feeding regimens, laboratory findings, and relevant clinical documentation.
• Provide resident, family, and staff education.
• Maintain accurate, timely documentation in accordance with facility policies and applicable standards.
• Support quality-improvement, survey-readiness, and regulatory-compliance activities.
