Job summary

Tri Care LLC in New York is seeking a Registered Dietitian to provide nutrition care in a long-term care setting. You will work with an interdisciplinary team to ensure high-quality nutrition services for residents.

Responsibilities include assessing residents, developing care plans, counseling families, and contributing to menus and weight monitoring. Proficiency with EMR systems like Sigma Care and PCC is required.

Qualifications

• Must be a Registered Dietitian.
• Experience in long-term care.
• EMR experience (Sigma Care, PCC).
• Strong verbal, writing, and organizational skills.

Responsibilities

• Collaborate with interdisciplinary team to ensure optimal resident care.
• Assess and develop nutrition care plans for residents.
• Participate in interdisciplinary care team meetings.
• Record assessments, care plans, MDS, progress notes in Sigma Care and PCC.
• Contribute to geriatrics menus.
• Provide nutrition counseling to residents and families; recommend diets and supplements.
• Knowledge of diets and tube feeding.
• Observe meals and monitor weight.

Duties and Responsibilities of the Registered Dietitian:

• Working with interdisciplinary team members to ensure optimal care is provided to all residents
• Assess, Develop and implement nutrition care plans to facilitate the provision of high quality and appropriate nutrition care to residents.
• Participate in interdisciplinary care team meetings
• Sigma Electronic assessments, care plans, MDS’s, progress notes, weight monitoring
• Geri Menu
• Provide nutrition counseling to residents and family, make diet & supplement recommendations
• Knowledge of diets and tube feeding
• Meal Observation

Requirements of the Registered Dietitian:

• Must have experience as a Registered Dietitian
• Must have experience in long term care
• Must have EMR (sigma care, PCC) experience
• Candidate must possess good verbal, writing, and organizational skills.
• Candidate must have strong computer knowledge
