Qualifications

Responsibilities

• Complete nutrition assessments, reassessments, and follow-ups for skilled nursing residents.
• Identify residents at nutritional risk and develop interventions.
• Monitor residents with weight changes, wounds, and metabolic conditions.
• Review weight trends, labs, meals, supplements, medications.
• Develop and update nutrition care plans and interventions.
• Provide recommendations on therapeutic diets and supplements.
• Communicate nutritional status and recommendations to the team.
• Document timely in the EMR and care plans.
• Participate in interdisciplinary care meetings and weight/ wound meetings.
• Provide nutrition education to residents/families.

Registered Dietitian – Skilled Nursing Facility | 16–24 Hours/Week | 50% Remote

RD Nutrition Consultants LLC is seeking a Registered Dietitian for a part-time position supporting a skilled nursing facility. This position offers 16–24 hours per week with approximately 50% of the work completed remotely.

We are looking for an RD who is comfortable working with the clinical and regulatory needs of a long-term care/skilled nursing population.

Schedule:

Flexible, Monday–Friday during normal business hours

Hours:

16–24 hours per week (approximately 50% remote)

Compensation:

$40.00–$45.00 per hour, based on experience

At least 1 year of clinical experience is preferred. Skilled nursing/long-term care experience is strongly preferred.

Responsibilities:

• Complete comprehensive nutrition assessments, reassessments, and follow-ups for skilled nursing residents in accordance with facility policies and applicable standards.
• Identify residents at nutritional risk and develop appropriate nutrition interventions and recommendations.
• Monitor residents with weight loss/gain, pressure injuries, altered skin integrity, poor oral intake, enteral nutrition, renal disease, diabetes, and other conditions affecting nutritional status.
• Review weight trends, laboratory values, meal intake, supplements, medications, and other relevant clinical information.
• Develop and update individualized nutrition care plans, goals, and interventions.
• Provide recommendations regarding therapeutic diets, oral nutrition supplements, food preferences, and other nutrition interventions.
• Communicate changes in nutritional status and recommendations to nursing, physicians/providers, dietary staff, and other members of the interdisciplinary team.
• Complete timely documentation in the facility’s electronic medical record.
• Participate in interdisciplinary care plan meetings, weight meetings, wound/skin meetings, and other clinical meetings as assigned.
• Provide nutrition education to residents and/or family members as appropriate.
• Collaborate with the Food Service Director/CDM and facility team regarding resident nutrition needs and concerns.
• Assist with regulatory compliance related to nutrition services and support the facility during survey preparation as needed.
• Maintain resident confidentiality and adhere to facility policies, procedures, and residents’ rights.
• Perform additional nutrition-related duties as assigned.

Requirements:

• Registered Dietitian credential through the Commission on Dietetic Registration (CDR).
• State licensure/certification as required.
• Clinical nutrition experience; skilled nursing or long-term care experience strongly preferred.
• Familiarity with nutrition assessment and care planning for geriatric and medically complex residents.
• Experience with electronic medical record/charting systems.
• Understanding of interdisciplinary care in a skilled nursing environment.
• Strong clinical judgment, organization, and time-management skills.
• Excellent written and verbal communication skills.
• Ability to work independently while collaborating effectively with the facility’s interdisciplinary team.
