Job summary

AHRC Nassau in Plainview, NY seeks a Registered Dietitian/Nutritionist to provide medical nutrition therapy and develop specialized diets for individuals, including those with diabetes, renal disease, and other conditions. The role involves conducting dietary assessments, updating plans, and collaborating with agency staff.

You will train residential staff on diet orders, document progress, and participate in supervisory sessions.

Qualifications

  • RD/ Nutritionist required.
  • NYS Certification mandatory.
  • 1+ year experience with I/DD population preferred.

Job description

Overview

To provide medical nutrition therapy which will enhance the health and well being of individuals.

Responsibilities
  • Provides medical nutrition therapy to individuals and agency staff. Develop specialized diets to aid in the management of medical conditions such as: diabetes, renal disease, celiac disease, hyperlipidemia and other medical conditions.
  • Conducts nutritional assessments including dietary history, medical history, anthropometric measurement, biochemical assessment and pharmaceutical assessment as needed to determine nutrition requirement.
  • Completes comprehensive nutritional assessments and follow-up progress notes. Documentations are completed according to nutrition department protocol
  • Recommends specialized diets to primary care physician. Obtains signed diet orders and trains residential staff regarding implementation of the diet orders
  • Makes and returns phone calls, emails and fax’s. Tracks lab work. Follows protocol from updating diet and sending out to appropriate team members
  • Provides for collateral intervention regarding the nutritional needs and health concerns of the individual according to the treatment plan.
  • Participates in scheduled supervisory sessions.
  • Assists in assignments as instructed by supervisor.
  • Performs other duties as requested by supervisor.
  • Ensure that work-related safety procedures are in place and are being adhered to by staff. Observance of an unsafe act or a potentially hazardous work environment must be immediately reported to supervisor and/or safety committee.

This position is primarily performed in an office environment and requires regular use of standard office equipment, including computers, telephones, printers, and scanners. Essential functions involve reviewing documents, entering, and retrieving information, and communicating with employees and other stakeholders. The employee may occasionally maintain a stationary work position for periods of time, move throughout the workplace, and lift or transport materials weighing up to 20 pounds. Employees must be able to perform the essential functions of the position, with or without reasonable accommodation.
