Advocate Health is seeking a Diabetes Education Specialist at Lakeshore Southpointe and Sinai locations in Wisconsin. The role focuses on delivering DSMES and MNT services to patients with diabetes, in inpatient or outpatient settings, and involves ongoing education and follow-up.

Current license or certification to practice as a Registered Nurse (RN) or Registered Dietitian, in the State of applicable state. Certified Diabetes Care and Education Specialist (CDCES) preferred. If not certified, you must obtain certification within two years preferred but not to exceed 3 years.
Degree in Nursing, Dietetics, or other related disciplines that meet the certification eligibility requirements for CDCES. If nursing degree, BSN preferred. Master's degree in Dietetics required if Dietician license obtained after 01/01/2024.
2 years of patient-oriented educational experience preferred with a broad range of knowledge on diabetes for age-appropriate population and its interrelation to co-existing health conditions.
