Job summary

The New Jewish Home Career Center is seeking a Registered Dietitian for full-time and part-time roles on our Manhattan campus. The position supports residents in long-term and sub-acute care with individualized nutrition care plans and education.

The role involves interdisciplinary collaboration, regulatory documentation, and comprehensive nutrition assessment to promote resident health outcomes.

Responsibilities

• Conduct nutrition assessments and provide clinical nutrition services for long-term and sub-acute care residents
• Complete accurate and timely medical record documentation, including MDS 3.0
• Develop, implement, and monitor individualized nutrition care plans
• Provide nutrition education to residents, patients, and family members as needed
• Participate in interdisciplinary care plan meetings
• Conduct meal rounds to ensure quality, safety, and resident satisfaction
• Collaborate with nursing, medical, and rehabilitation staff to support resident care goals
