Job description

Position Summary: We are seeking a PRN Hybrid Registered Dietitian to join our team at a skilled nursing facility. This position offers flexibility with 16 hours per week, including in-house hours and remote work as needed. The successful candidate will be responsible for providing nutrition care for residents, assessing their nutritional needs, and developing individualized care plans to improve and maintain their health.

Key Responsibilities:

  • Conduct thorough nutrition assessments for new admissions, as well as ongoing assessments for current residents.
  • Develop, implement, and monitor individualized nutrition care plans based on residents' medical needs and goals.
  • Provide dietary counseling to residents, their families, and staff on nutrition-related matters, including special diets, weight management, and chronic disease management.
  • Collaborate with the interdisciplinary care team to provide optimal care and support for residents.
  • Ensure accurate documentation of nutrition-related care and recommendations in residents' medical records.
  • Assist with menu planning and the development of therapeutic diets in accordance with residents' needs and facility guidelines.
  • Provide education and training for nursing and dietary staff on nutrition-related topics.
  • Participate in quality improvement initiatives to enhance nutrition services and overall resident care.
  • Adhere to all regulatory standards, including those set by the CDC, state health departments, and the facility’s policies and procedures.
  • Perform other duties as assigned, including remote consultations when appropriate.

Qualifications:

  • Bachelor’s degree in Dietetics, Nutrition, or a related field.
  • Registered Dietitian (RD) credential through the Commission on Dietetic Registration (CDR).
  • State licensure or certification, if required by the state.
  • Experience working in a skilled nursing facility or long-term care setting preferred.
  • Strong knowledge of nutrition assessment, medical nutrition therapy, and therapeutic diets.
  • Ability to work independently and as part of an interdisciplinary team.
  • Excellent communication skills, with the ability to educate residents, families, and staff.
  • Ability to manage a flexible work schedule with both in-house and remote hours.
  • Proficiency with electronic medical records (EMR) and other relevant software tools.
