Join Our Team as a Registered Dietitian (Hybrid PRN) at Premier Healthcare, LLC

We are seeking a PRN Hybrid Registered Dietitian to join our skilled nursing facility team. This flexible position offers 16 hours per week, combining in-house and remote work as needed. The successful candidate will provide nutrition care, assess residents' nutritional needs, and develop personalized care plans to support their health.

Key Responsibilities
  1. Conduct comprehensive nutrition assessments for new and current residents.
  2. Develop, implement, and monitor individualized nutrition care plans based on medical needs and goals.
  3. Provide dietary counseling to residents, families, and staff on nutrition-related topics, including special diets, weight management, and chronic disease management.
  4. Collaborate with the interdisciplinary care team to deliver optimal resident care.
  5. Document nutrition-related care and recommendations accurately in residents' medical records.
  6. Assist in menu planning and creating therapeutic diets tailored to residents' needs.
  7. Educate and train nursing and dietary staff on nutrition topics.
  8. Participate in quality improvement initiatives to enhance nutrition services.
  9. Ensure compliance with all regulatory standards, including CDC, state health departments, and facility policies.
  10. Perform additional duties, including remote consultations as needed.
Qualifications
  • Bachelor’s degree in Dietetics, Nutrition, or related field.
  • Registered Dietitian (RD) credential from the CDR.
  • State licensure or certification, if required.
  • Experience in a skilled nursing or long-term care setting is preferred.
  • Strong knowledge of nutrition assessment, medical nutrition therapy, and therapeutic diets.
  • Ability to work independently and collaboratively within a team.
  • Excellent communication skills for educating residents, families, and staff.
  • Flexible work schedule with both in-house and remote hours.
  • Proficiency with EMR and relevant software tools.
