Job summary

Healthcare Services Group (HCSG) is seeking a Hybrid Registered Dietitian for a part-time role (12-20 hours per week). The position supports clinical nutrition at long-term care facilities, with a Monday–Friday schedule and flexible hours.

You will assess residents' nutrition needs, develop care plans, and provide education to residents, families, and staff, collaborating with interdisciplinary teams to improve health outcomes while ensuring CMS and state compliance.

Qualifications

• RD certification and licensure in good standing.
• Strong knowledge of nutrition care regulations and CMS/state guidelines.
• Ability to educate residents, families, and staff on nutrition.
• Excellent organizational and communication skills.

Responsibilities

• Assess the clinical nutrition needs of residents and document plans in the EMR.
• Develop, implement, and evaluate individualized care plans based on clinical data and weight trends.
• Provide nutrition education and counseling to residents, families, and staff.
• Collaborate with interdisciplinary teams to improve health outcomes.
• Ensure adherence to regulatory requirements and company standards.
• Provide coverage across multiple buildings as needed.

Responsibilities

• Assess the clinical nutrition needs of residents/patients and develop and document the plan within the electronic medical record.
• Develop, implement, and evaluate individualized care plans based on clinical findings, lab results, weight trends, intake, and comorbidities
• Provide nutrition education and counseling to residents/patients, families, and staff.
• Collaborate with interdisciplinary teams to determine a plan of care that improves residents'/patients’ overall health and well-being.
• Ensure adherence to CMS, State, and Federal regulations and company standards.
• Provide coverage in multiple buildings based on company needs, and as desired.
• All other duties as assigned.

Qualifications

• Registered by the Commission on Dietetic Registration. **
• Hold a Certification/License in good standing within the state of practice.
• Knowledge of the state survey process and compliance with nutrition care regulations (preferred, but not required).
• Strong organizational and communication skills.
• Maintain a positive attitude and effectively communicate with building occupants, administration, and HCSG leadership to perform daily tasks.
• Compliance with COVID-19 vaccination policies as required by state, local, and/or customers.
• If multi-site registered dietitian, must have valid Driver's License

**RD-Eligible and provisional state licensure accepted in certain locations
