You will work with facility teams to assess residents, develop nutrition plans, and ensure high-quality care through rounds, menu sign-offs, and EMR documentation.

State licensure is required; CDR credential or eligibility with verification is expected.

Qualifications

• Strong communication skills and effective teamwork are required.
• State licensure as required varies by state; CDR must be current.

Responsibilities

• Provide compassionate care focused on residents' nutritional needs.
• Collaborate with dietitians and clinical teams to deliver personalized nutrition plans.
• Complete clinical assessments, chart in EMR, and participate in care meetings.
• Conduct meal rounds, sanitation audits, and sign off on dietary menus.
