The Registered Dietitian will provide evidence-based nutrition assessment, counseling, and medical nutrition therapy across the lifespan in a multidisciplinary setting. Located in Connecticut, the role involves collaboration with physicians and care teams to improve health outcomes, with a focus on chronic disease management and prevention.

Qualifications

  • Minimum 3 years of experience required.
  • Master’s degree preferred in Nutrition, Dietetics, or related field.
  • Certified and enrolled dietitian-nutritionist under CMAP; Licensed Dietitian/Nutritionist in Connecticut.
  • Current CPR certification.
  • Experience in primary care, community health, FQHC, or school-based health center.
  • Bilingual, Spanish speaking, preferred.

Responsibilities

  • Conduct comprehensive nutrition assessments and develop individualized care plans.
  • Provide medical nutrition therapy for chronic conditions across the lifespan.
  • Review and approve meal plans in residential programs; deliver nutrition education.

Summary:

The Certified and enrolled Dietitian provides evidence-based nutrition assessment, counseling, education, and medical nutrition therapy (MNT) to patients across the lifespan in a multidisciplinary healthcare setting, and school-based health centers. The dietitian works collaboratively with physicians, advanced practice providers, nurses, behavioral health clinicians, care coordinators and community health workers to improve health outcomes, manage chronic disease, and promote disease prevention for a diverse patient population.

Essential Duties & Responsibilities:
  • Conduct comprehensive nutrition assessments using evidence-based standards across the lifespan.
  • Develop individualized nutrition care plans based on patients’ medical diagnosis, cultural preferences, socio-economic factors, social determinants of health screenings, and health goals.
  • Provide medical nutrition therapy (MNT) for conditions including Diabetes and Diabetes prevention, Hypertension, Hyperlipidemia, obesity and weight management, chronic kidney disease, gastrointestinal disorders, food allergies, and intolerances, and pediatric nutrition and malnutrition.
  • Review and approval of meal plans in residential programs.
  • Deliver individual and group nutritional education sessions.
  • Collaborates with multidisciplinary care teams and accepts referrals from qualified providers for MNT.
  • Participate in chronic disease management programs, and quality improvement initiatives.
  • Document patient encounters in the electronic health record in accordance with organizational, state and federal guidelines and comply with standard billing practices.
  • Screen patients for food insecurity, and connect them with community resources, and nutritional assistance programs.
  • Support patients with lifestyle modification, behavioral changes, and goal setting using motivational interviewing techniques.
  • Maintain productivity standards while ensuring high-quality patient centered care.
  • Stay current with clinical guidelines, continuing education, and best practices.

*All agency staff are required to attend all mandatory department/agency meetings and trainings*

Monday - Friday, 8:30 AM - 5:00 PM

Requirements
Education &/or Experience:

Minimum of 3 years of experience.
