Job summary

Skilled Nursing and Rehabilitation Center in Union County, NJ is seeking an experienced Registered Dietitian to join our healthcare team. The RD will conduct nutritional assessments, develop care plans, and provide medical nutrition therapy.

You will participate in MDS, QA/QAPI, ensure regulatory compliance, and collaborate with nursing, physicians, and dietary staff to optimise resident nutrition and quality of care.

Qualifications

• RD/RDN credential required (CDR).
• Bachelor's degree in Dietetics, Nutrition, or related field.
• Minimum 2 years in SNF or LTC preferred.
• Strong knowledge of clinical nutrition and MDS care planning.
• Familiarity with CMS regulations in SNFs.
• Experience with SigmaCare and GeriMenu preferred.
• Excellent clinical judgment, organization, and attention to detail.

Responsibilities

• Conduct comprehensive nutritional assessments for residents at admission and during care.
• Develop, implement, and monitor individualized nutrition care plans based on medical conditions and preferences.
• Provide Medical Nutrition Therapy to residents with diabetes, renal disease, wounds, and malnutrition.
• Participate in MDS assessment process and care plan meetings.
• Monitor nutritional status, weight changes, and dietary intake.
• Coordinate with nursing, physicians, and therapy teams to ensure resident-centered nutrition care.
• Ensure meals, supplements, and diets meet physician orders and regulatory standards.
• Provide nutrition education to residents, families, and staff.
• Oversee dietary operations to maintain quality, safety, and compliance.
