Qualifications

Responsibilities

  • Get to know your patients through initial and ongoing assessments and become their nutritional detective.
  • Evaluate and track patients' nutritional status and identify opportunities to help them thrive.
  • Create personalized diet plans and share nutrition expertise with patients, caregivers, and the care team.
  • Educate patients and staff to improve nutrition knowledge and outcomes.
  • Collaborate with hospitals, home health, and other partners to coordinate care.
  • Review monthly lab results with patients and discuss implications for nutrition plans.
  • Suggest nutrition-related adjustments to medications and supplements as needed.
  • Participate in interdisciplinary care meetings and quality improvement initiatives.
  • Mentor Dietetic Interns when possible and contribute to ongoing professional development.

Job Summary:

Responsible for all aspects of nutritional needs of dialysis patients. Will strive to fulfill the organization’s mission, vision, values, philosophy and goals. Works closely with the interdisciplinary team members in order to care for the dialysis patient. Develops a close working relationship with other dietitians in order to facilitate information flow and educational efforts. Reports to designated supervisor.

Duties & Responsibilities:

  • Get to know your patients (and their support systems!) through initial and ongoing assessments—think of it as becoming their nutritional detective
  • Evaluate and track each patient's nutritional status like a pro, spotting trends and opportunities to help them thrive
  • Create personalized diet plans and share your nutrition expertise with patients, their caregivers, and the care team
  • Become the go-to nutrition educator, empowering patients and staff with knowledge that makes a real difference
  • Build bridges with other healthcare partners—hospitals, extended care facilities, home health agencies, senior centers, and "Meals on Wheels" programs—keeping the conversation flowing
  • Keep tabs on how well patients are sticking to their nutrition plans and celebrate their wins
  • Recommend adjustments to phosphorus binders, renal vitamins, stool softeners, oral iron products, cinacalcet, vitamin D analogs, and calcium supplements based on patient needs
  • Review monthly lab results with patients, turning numbers into meaningful conversations
  • Suggest creative nutritional solutions—from alternate feeding routes to tube feedings and supplemental options—and monitor how they're working
  • Jump into action during critical moments, responding swiftly to urgent situations like hyperkalemia or panic values
  • Stay sharp on drug-nutrient interactions and how medications impact your patients' nutrition
  • Collaborate with the interdisciplinary team in patient care meetings, ensuring your voice is heard and documented
  • Roll up your sleeves in unit quality improvement initiatives—your ideas matter!
  • Actively participate in GHS dietitian meetings and committees, contributing to the bigger picture
  • Hunt down and secure nutritional resources that help your patients succeed
  • Be an integral part of the healthcare team—making referrals, sharing knowledge, and championing the use of educational materials while creating new ones
  • Lead by example as a professional role model for colleagues and students
  • Invest in your own growth through conferences, professional memberships, and continuing education—especially in nephrology
  • Maintain your ADA registration and stay current in the nephrology field
  • Participate in and help facilitate kinetic modeling to optimize patient outcomes
  • May contribute to protocol management and development
  • Make thoughtful recommendations to patients and the care team based on comprehensive nutritional assessments
  • Mentor Dietetic Interns when the opportunity arises, passing along your expertise
  • Take on additional responsibilities as needed to support the team
Qualifications

Education & Experience:

  • Requires Bachelor of Science in Nutrition
  • Requires completion of an approved dietetic internship
  • ADA Registered and minimum of 1 year professional experience as RD in clinical nutrition required
