Job summary

Tanner Health is seeking an Registered Dietitian / Certified Diabetes Care and Education Specialist (RD, CDCES) to provide nutrition and diabetes care services. The role focuses on Medical Nutrition Therapy (MNT), DSMES, wellness coaching, and community health education, using established guidelines and the Nutrition Care Process to improve patient outcomes.

Qualifications

Comprehensive knowledge of MNT, DSMES, Nutrition Care Process, guidelines, chronic disease prevention and management, behavior change theories, population health principles, and health coaching strategies.
Strong communication and presentation skills.

Responsibilities

Provide Medical Nutrition Therapy (MNT) and Diabetes Self-Management Education and Support (DSMES) using the Nutrition Care Process.
Collaborate with interdisciplinary teams to assess, develop, implement, monitor and evaluate individualized nutrition and diabetes care plans.
Support wellness coaching, Food as Medicine initiatives, and community health education.
Participate in community outreach and employee wellness initiatives.
