Job summary
Elica Health Centers is seeking a Registered Dietitian to provide Medical Nutrition Therapy and lifestyle counseling for patients with chronic conditions, including diabetes, hypertension, and obesity. You will perform nutrition assessments and develop individualized care plans based on medical history and clinical data.

- Must hold a Registered Dietitian credential and be licensed.
- Experience with nutrition care plans for chronic conditions.
Responsibilities
- Provide Medical Nutrition Therapy and lifestyle counseling for chronic conditions (Diabetes, Hypertension, Obesity).
- Perform comprehensive nutritional assessments and develop individualized care plans.
- Develop nutrition education curricula and translate materials as needed.
- Collaborate with clinical/non-clinical staff to integrate nutrition education into patient care.
- Monitor patient outcomes and ensure continuity of care.
