Job summary

Outreach Community Health Centers is seeking a Dietitian to lead clinical and community nutrition initiatives under the Associate Medical Director. The role develops individualized care plans, educates patients and groups, and supports nutrition programs designed to reduce disparities and improve health outcomes.

You will collaborate with clinicians, nurses, and staff, coordinate education, and ensure documentation and billing are complete while adhering to HIPAA and regulatory standards.

Qualifications

• Bachelor’s degree in Dietetics and Wisconsin licensure required; RD credential required at time of employment.
• 1–2 years clinic or hospital experience preferred.
• Proficiency in MS Office, EMR, and nutrient analysis software; strong written, oral, and electronic communication skills.

Responsibilities

• Provide evidence-based, patient-centered nutrition counseling.
• Develop and implement individualized care plans with clinicians and the multidisciplinary team.
• Coordinate nutrition education programs and materials for diverse populations.
