Advocate Health Care seeks a nutrition professional to collaborate with physicians and care teams to deliver nutrition services across health centers. You will develop care plans, counsel individuals and groups, and document patient progress.
The role requires licensure as a dietitian in Illinois, ADA registration, CPR within 60 days, and at least one year of clinical nutrition experience. Travel to multiple centers is expected, and you will participate in education and outreach activities,

Must be able to respond to the age appropriate psychological and physical needs of patients.
Understands growth and development needs of the age group with which the associate interacts.
Knowledge, Skills & Abilities Required: Good interpersonal and communication skills.
Accurate with follow through.
ADA registration.
Illinois license as a licensed dietitian required.
CPR certification within 60 days of hire.
Frequent travel to various centers.
Ability to work varying shifts to meet center needs.
If position has direct patient care or direct patient contact the following lifting requirement supersedes any previous lifting requirement effective 06/01/2015.
Ability to lift up to 35 pounds without assistance. For patient lifts of over 35 pounds, or when patient is unable to assist with the lift, patient handling equipment is expected to be used, with at least one other associate, when available.
