Job summary

Advocate Health Care seeks a nutrition professional to collaborate with physicians and care teams to deliver nutrition services across health centers. You will develop care plans, counsel individuals and groups, and document patient progress.

The role requires licensure as a dietitian in Illinois, ADA registration, CPR within 60 days, and at least one year of clinical nutrition experience. Travel to multiple centers is expected, and you will participate in education and outreach activities,

Qualifications

• Bachelor's degree in Clinical Nutrition or related field.
• Illinois Dietitian license is required.
• ADA registration is required.
• CPR certification within 60 days of hire.
• Minimum of 1 year clinical nutrition experience.

Responsibilities

• Develops nutrition care plans per ADA guidelines or provider recommendations.
• Provides individual and group dietary counseling.
• Reviews medical records and discusses care plans with staff.
• Documents counseling and patient understanding in SOAP notes.
• Maintains communication with patients and families.
• Orders lab tests per Advocate Health Care protocol.
• Teaches nutrition classes and mentors interns.
• Develops educational materials and displays.
• Provides in-service training to associates.
• Guides clinical experience of dietary student interns.
• Participates in committees as assigned.
• Serves as a resource to physicians regarding nutrition.
• Researches, plans, and develops curriculum.

• Promotes National Nutrition Month in health centers.

Professional Development responsibilities

• Maintains active licensure and registration status.
• Keeps abreast of current standards of care via literature, educational events and internal and external resources. Licensure: Dietitian Nutritionist Education/Experience Required: Bachelors Degree in Clinical Nutrition.

One Year Clinical Nutrition Experience Required.

Must be able to respond to the age appropriate psychological and physical needs of patients.

Understands growth and development needs of the age group with which the associate interacts.

Knowledge, Skills & Abilities Required: Good interpersonal and communication skills.

Accurate with follow through.

ADA registration.

Illinois license as a licensed dietitian required.

CPR certification within 60 days of hire.

Physical Requirements And Working Conditions

Frequent travel to various centers.

Ability to work varying shifts to meet center needs.

If position has direct patient care or direct patient contact the following lifting requirement supersedes any previous lifting requirement effective 06/01/2015.

Ability to lift up to 35 pounds without assistance. For patient lifts of over 35 pounds, or when patient is unable to assist with the lift, patient handling equipment is expected to be used, with at least one other associate, when available.
