Job Description: $10,000 Sign on Bonus for eligible applicants Work Schedule: 5 days a week, will need to rotate coverage on weekends (usually work 1 weekend a month)
Conducts comprehensive nutrition assessments by evaluating medical history, anthropometric data, nutrition-focused physical findings, dietary intake, and malnutrition risk to develop individualized care plans. Provides evidence-based nutrition interventions, counseling, education, and care management for patients with acute and chronic disease states, utilizing culturally sensitive and health literacy-appropriate approaches. Monitors, evaluates, and adjusts nutrition care plans to achieve clinical nutrition goals, optimize patient outcomes, and support recovery and disease management. Collaborates with physicians, nurses, and interdisciplinary healthcare teams during patient care rounds to develop, implement, and coordinate nutrition plans of care, including nutrition orders when authorized. Maintains accurate electronic health record documentation while supporting quality improvement, research initiatives, staff development, and the training of dietetic interns, technicians, and new team members.
