Job summary

Sweetwater Care is seeking a Registered Dietitian to provide evidence-based medical nutrition therapy across our skilled nursing, independent living, and assisted living settings. You will work with an interdisciplinary team to promote optimal nutritional status and resident-centered care.

Responsibilities include completing assessments and care plans per CMS, monitoring weight and labs, and documenting accurately for MDS and care planning.

Qualifications

• Bachelor’s degree or higher in Nutrition or Dietetics.
• Registered Dietitian Nutritionist (RDN) credential.
• Knowledge of CMS regulations and long‑term care standards.
• SNF experience preferred but not required.

Responsibilities

• Provide medical nutrition therapy for residents with complex medical conditions.
• Monitor weight trends, intake, labs, and nutrition‑related risks; recommend interventions as needed.
• Ensure compliance with federal, state, and facility regulations and guidelines.
• Accurately document assessments and contribute to MDS and care planning processes.
• Collaborate with the interdisciplinary team to support resident‑centered care.
• Participate in quality improvement activities related to nutrition and hydration.
• Provide nutrition education to residents, families, and staff as appropriate.

This role ensures compliance with federal and state regulations, promotes optimal nutritional status, and supports resident‑centered care through collaboration with the interdisciplinary team.
