JOB SUMMARY:

We are seeking a part-time Dietitian to join our dedicated healthcare team and provide comprehensive nutritional care to our residents. The Dietitian plays an essential role in assessing nutritional needs, developing individualized nutrition care plans, providing medical nutrition therapy, and collaborating with the interdisciplinary team to promote optimal resident health and well-being. RESPONSIBILITIES:
- Conduct comprehensive nutritional assessments for residents upon admission and as required thereafter, identifying nutritional risks, deficiencies, and individualized dietary needs.
- Develop, implement, and monitor individualized nutrition care plans based on residents’ medical conditions, nutritional status, preferences, and goals of care.
- Provide medical nutrition therapy and make appropriate recommendations regarding therapeutic diets, supplements, and other nutritional interventions in accordance with physician orders and applicable regulations.
- Monitor residents’ nutritional status, including weight trends, meal intake, hydration, laboratory values, skin integrity, and other relevant clinical indicators.
- Participate in interdisciplinary care plan meetings and collaborate with nursing, physicians, speech therapy, rehabilitation, food services, and other members of the healthcare team.
- Provide nutrition counseling and education to residents and their families or responsible parties regarding therapeutic diets, nutritional needs, food choices, and healthy eating practices.
- Review and evaluate residents experiencing significant weight changes, poor intake, pressure injuries, swallowing concerns, or other conditions that may affect nutritional status.
- Make recommendations regarding nutritional supplements, fortified foods, modified diets, and other interventions to support residents’ nutritional and hydration needs.
- Collaborate with the Dietary/Food Services Department to ensure menus and meal services are consistent with residents’ nutritional requirements, therapeutic diets, cultural preferences, and individual needs.
- Review menus and therapeutic diet modifications to ensure nutritional adequacy and compliance with applicable standards and regulations.
- Monitor resident response to nutritional interventions and update care plans and recommendations as clinically appropriate.
- Maintain accurate, timely, and comprehensive documentation in residents’ medical records, including assessments, care plans, progress notes, recommendations, and follow-up.
- Participate in quality assurance and performance improvement initiatives related to nutrition, weight management, hydration, food service, and resident outcomes.
- Assist with regulatory compliance and survey readiness by maintaining appropriate nutritional documentation and ensuring dietary practices meet applicable federal and state requirements.
- Stay current with evidence-based nutrition practices, regulatory requirements, and developments in clinical nutrition and long-term care.
- Serve as a knowledgeable resource to residents, families, nursing staff, dietary staff, and other members of the interdisciplinary care team on nutrition-related matters.
QUALIFICATIONS:
- Registered Dietitian (RD) credential through the Commission on Dietetic Registration (CDR).
- Current Connecticut Dietitian-Nutritionist certification as required by applicable state regulations. Connecticut recognizes current CDR Registered Dietitian certification as one pathway to Connecticut certification.
- Bachelor's degree in Dietetics, Nutrition, or a related field from an accredited college or university.
- Previous clinical nutrition experience preferred, with experience in long-term care or skilled nursing strongly preferred.
- Knowledge of medical nutrition therapy, therapeutic diets, nutrition assessment, and nutrition care planning.
- Experience working with older adults and individuals with complex medical and nutritional needs preferred.
- Strong interpersonal and communication skills; ability to work effectively with residents, families, and an interdisciplinary healthcare team.
- Excellent organizational and time-management skills, with the ability to prioritize multiple residents and clinical responsibilities.
- Strong clinical judgment and attention to detail.
- Ability to maintain accurate and timely clinical documentation.
- Knowledge of applicable federal and state regulations related to nutrition and dietary services in skilled nursing facilities.
- Ability to work collaboratively with nursing, dietary, rehabilitation, medical, and administrative staff.
- Be able to read, write and understand English.
