Job summary

Solaris HealthCare Bayonet Point seeks a qualified Dietitian to coordinate residents' nutritional care, perform assessments, and develop individualized care plans in a long-term care setting.

You will document dietary information, monitor residents’ nutrition, and advise on diets, supplements, and weight management while collaborating with interdisciplinary teams. AHCA screening is a requirement and licensure is essential.

Qualifications

• Registration with the Academy of Nutrition and Dietetics and state licensure.
• At least 1 year of experience in long-term care.
• Florida AHCA Clearinghouse background screening required.

Responsibilities

• Coordinate nutritional care of residents by completing nutritional assessments and developing care plans.
• Document dietary information in accordance with policies and regulations.
• Visit residents to assess effectiveness and update food preferences and intake.
• Provide guidance on liberalized diets, supplements, and weight management.
• Oversee and participate in dining room operations and nutrition reviews.

Purpose of Your Job Position

The primary purpose of your job position is to coordinate nutritional care of residents by completing nutritional assessments, developing and implementing care plans, and documenting dietary information about residents, in accordance with current federal, state, and local standards, guidelines and regulations, and Solaris HealthCare’s established policies and procedures.

  • Completes nutritional assessment, MDS, CAA’s and care plans.
  • Completes progress notes.
  • Assesses and completes required documentation on residents nutritionally at risk according to time frames for assessments.
  • Visits residents routinely to determine effectiveness of interventions and to update food preferences and evaluate food intake.
  • Promotes and assists in implementation of liberalized diets.
  • Follows house supplement procedure recommending enhanced foods before supplements. Evaluates the effectiveness of supplements on a routine basis.
  • Reviews weight records routinely and communicates variances to interdisciplinary team.
  • Makes enteral feeding recommendations that are consistent with procedures.
  • Evaluates and monitors TPN.
  • Ensures timely follow-up on physician recommendations.
  • Communicates resident nutrition and hydration relevant changes in condition during the 24-hour Clinical Report/Stand-Up Meeting.
  • Uses current standards of dietetic practice in the implementation of nutritional care.
  • Serves as Dining Room Coordinator, Dining Room Monitor as required.
  • Oversees and ensures accuracy of nutritional reviews done by the Dietetic Technician and the Food Service Director.

Education

Registration with the Academy of Nutrition and Dietetics and licensed by the state where he/she practices, if appropriate.

Experience
