Job summary

RCM Healthcare Services is seeking a Registered Dietitian for the PACE program in Evansville, IN. You will assess participants’ nutritional status, develop individualized nutrition interventions, and collaborate with the interdisciplinary team to deliver participant-centered care for frail elders.

You will provide nutrition counseling, monitor weight and hydration, coordinate meals and supplements, and document care plans in accordance with PACE regulations.

Qualifications

• Bachelor’s degree in nutrition, dietetics, or related field from an accredited college.
• Registered Dietitian credential (Commission on Dietetic Registration).
• Strong clinical assessment and nutrition care planning skills.
• Knowledge of geriatric nutrition, chronic disease management, and drug-nutrient interactions.

Responsibilities

• Schedule/shift: Days, 40 hours per week.
• Conduct comprehensive nutritional assessments at enrollment and follow-ups.
• Develop and update individualized nutrition care plans with the team.
• Provide nutrition counseling to participants, caregivers, and families.

Position Summary

As a Registered Dietitian within a PACE (Program of All-Inclusive Care for the Elderly) organization, you will assess participants’ nutritional status, develop individualized nutrition interventions, and collaborate with the interdisciplinary team to deliver comprehensive, participant-centered care. This role plays a critical part in supporting frail and elderly participants through nutrition counseling, care planning, and ongoing monitoring to promote health, independence, and quality of life.

• Key Duties:

• Conduct comprehensive nutritional assessments at enrollment and periodic reassessments per PACE regulations


• Develop and update individualized nutrition care plans in collaboration with the interdisciplinary team


• Provide nutrition counseling and education to participants, caregivers, and families


• Monitor clinical indicators including weight, hydration, intake, swallowing/chewing issues, and drug-nutrient interactions


• Recommend and coordinate nutrition-related interventions such as meal services and supplements


• Participate in interdisciplinary team meetings, care conferences, and quality improvement initiatives


• Maintain accurate documentation and ensure compliance with safety and infection control protocols
