Job summary

Rocky Mountain Health Care Services (RMHCS) seeks a Registered Dietitian to join the Interdisciplinary Team. The RD will complete nutritional assessments, determine nutrition risk, provide education and counseling, and develop interventions for participants in long‑term care.

You will collaborate with care providers, participate in team meetings, assist with community food resources, and help implement nutrition policies.

Qualifications

• Bachelor's degree in food science, nutrition, or dietetics required.
• CDR-registered RD certification required.
• 1 year in long-term care preferred; elderly population experience desired.
• Strong knowledge of clinical nutrition and therapeutic diets.
• Proficient with computer and EHR systems.
• Excellent communication and relationship-building with participants and staff.

Job description

The Registered Dietitian (RD) is an essential member of the Interdisciplinary Team and is responsible for a comprehensive knowledge of clinical nutrition and therapeutic diets. The RD completes participant nutritional assessments and reassessments as needed, determines nutrition risk status, provides nutrition education and counseling, and develops interventions to support nutrition needs of participants. Participates in program development, quality improvement and other projects and committees as assigned.

ESSENTIAL JOB FUNCTIONS:

• Completes participant assessments at enrollment and reviews nutrition status at least biannually
• Identifies nutrition risk concerns and collaborates with the IDT, participants and care providers in developing goals and interventions
• Provides nutrition education and counseling to participants, family and/or caregivers based on individual plans of care in accordance with Academy of Nutrition and Dietetics standards
• Maintains required certifications/registrations
• Determines and monitors therapeutic diets, special dietary modifications and communicates concerns with interdisciplinary team
• Serves as a member of the Interdisciplinary Team (IDT) to develop a comprehensive plan of care for each participant
• Assists participants with community food resources including Meals on Wheels, congregate meal sites, commodity foods, and coordination/communication with Project Angel Heart
• May be a resource for Kitchen and Day Center staff, oversees dining service needs, monitors meal service, special diet adherence, and addresses nutritional concerns with serving staff
• Assists in implementing nutrition/dietary policies and procedures
• Develops and maintains relationships with contractors and staff in contracted facilities to foster communication around nutrition care and interventions
• Provides nutrition education to staff and community as requested
• Monitors weights, medical diagnosis, changes in chewing, swallowing, hydration, mood changes, intake, other health and environment changes and report changes to interdisciplinary team members
• Participates in care team meetings, participant care conferences and family meetings
• Manages nutrition supplement products provided by Rocky Mountain Health Care Services
• Other duties as assigned

Qualifications

• Bachelors Degree from and an accredited college or university in food science, nutrition, and/or dietetics is required
• Certified by the Commission on Dietetic Registration (CDR) as a Registered Dietitian
• Minimum of 1 year experience in long-term care preferred, one year experience working with a frail or elderly population highly desired
• Comprehensive knowledge of clinical nutrition and therapeutic diets
• Skilled in medical terminology, including knowledge of the appropriate documentation for completing written assessments
• Skilled in the use of computer and EHR system
• Skilled in establishing and maintaining effective working relationships with participants, co-workers, vendors, medical staff, and the public
