Job Status: Part Time or Full Time

Reports To: Director of Health Education

Grade/Level: Bachelors;

Registered & Licensed Dietitian

Amount of Travel Required: 50%

Work Schedule:

Positions Supervised: None

8 a.m. to 5:30 p.m. Monday, Tuesday, Wednesday, and Thursday. 8 a.m. to 12:00 p.m. on Friday. However, additional hours may be required to complete duties.


POSITION SUMMARY:

The Registered Dietitian is responsible for the implementing and support of educational programs for patients regarding nutrition and healthy living techniques. Nutritional care will be provided by the Registered Dietitian through medical nutrition therapy assessments and MSHI’s diabetes self-management education and support program. The Registered Dietitian is involved in continuous Quality Improvement practices and measures as set by Mainline Health Systems, and in which the practice focus is based on a Patient Centered Medical Home model of care.


SPECIFIC DUTIES AND RESPONSIBILITIES:

  • Collaborate in the development and implementation of a Medical Nutrition Therapy program to aid patients in healthier lifestyle changes;
  • Facilitates various formats of multidisciplinary health education classes for patients with chronic diseases and their families; takes into consideration health literacy needs;
  • Promotes healthy living through eating well by designing, implementing, and teaching nutrition practices and programs;
  • Assess nutritional needs and current health plans to develop and implement medical nutrition therapy for MHSI patients;
  • Integrates skills and knowledge of pathophysiology, epidemiology, clinical management, cardiometabolic conditions and self-management of diabetes into clinical practice.
  • Provides quality diabetes self-management education in individual and group settings based on assessed needs. Uses appropriate teaching techniques that are sensitive to the learning preferences of the person with prediabetes or diabetes;
  • Follows the ADCES Diabetes Education Accreditation Program requirements which are based on the National Standard of Diabetes Self-Management and Support Guidelines;
  • Advocates for and supports technology-enabled diabetes education and care, in individual and population health diabetes services;
  • Communicates with primary and specialty care providers as needed. Consults with appropriate members of the health care team to promote optimal nutrition and diabetes care;
  • Serves as part of the care team in developing person-centered diabetes plans;
  • Collaborates with community partners to establish and maintain on-going support options.
  • Assist with marketing and community outreach to promote educational resources and classes;
  • Addresses referrals in a timely manner;
  • Documents all individual contacts/visits in the EHR according to MHSI and Accreditation Program guidelines in a timely manner;
  • Provide quarterly reports and outcome reports for patients served;
  • Prepare visual aids, health education outlines, and other materials used for teaching and demonstration;
  • Promote customer-driven excellence as a driver of organizational services and goals.
  • Performs all job duties in compliance within MHSI policies and procedures and HIPAA.
  • Other duties as assigned.

SKILLS & ABILITIES:

Education
Bachelor of Science in Nutrition/Dietetics

Credentialed by the Commission on Dietetic Registration

Experience
Minimum 2-year experience in nutrition and facilitation of diabetic classes preferred.

Computer Skills
Proficient utilizing standard office software applications and email.

Certificates & Licenses
Registered Dietitian
Board Certified Diabetic Educator preferred.

Other Requirements
Ability to handle sensitive and extensive confidential information
Knowledge of HIPAA
Bilingual preferred, but not necessary.
Willingness to attend and/or complete continuing education related to this position and/or any other topics as deemed necessary to MHSI.
Excellent verbal and written communication skills
Must be able to work with a diverse patient population and have exceptional customer service skills
The ability to work in a constant state of alertness and in a safe manner.

The Registered Dietitian reports to, is supervised, and evaluated by the Director of Health Education.

EVALUATION:

The evaluation of work performance will be ongoing and will be carried out by the Director of Health Education and/or Chief Quality/Compliance Officer. It will include specific duties and responsibilities of this position description plus employee attitude and general working behavior. Formal evaluation will be performed annually but may be initiated at other times by the Director of Health Education and/or Chief Quality/Compliance Officer.
