The role requires RD credentials (CDR) and NY licensure, with preferred senior living experience.

Qualifications

• Credentialed as a Registered Dietitian by the CDR.
• Licensed by the NY State Dietetics Licensing Board.
• One year of senior living experience preferred.
• Proficient with Microsoft Office, EMR, and nutrient analysis programs.

Responsibilities

Job description

Sapphire Rehabilitation of Northtowns an innovative skilled nursing facility, specializes in caring for the elderly is seeking a Registered Dietician to join our team of professional care givers TheRegistered Dieticianwill be responsible for assessing the nutritional status of patients and recommending the appropriate nutrition therapy as it pertains to the patients’ nutritional needs. TheRegistered Dieticianwill also be responsible in assisting the dietary staff in creating high quality meals and the delivering of a meaningful dining experience for clients in a long tern skilled nursing facility.

Responsibilities:

• Interviews patient and/or family for nutritional history, obtains patient food preferences, assesses patients, and documents in patients’ medical record.
• Conducts nutritional assessment, develops and implements patient specific care plans, and assists in recommending nutrition care plans to medical staff for review and consideration.
• Assists in recommending therapeutic diet modifications and/or supplemental feedings to physicians for patients, and ensures accuracy of diet order received by checking against physician’s order.
• Calculates nutrient content of tube feeding and dietary intakes as ordered by physicians.
• Provides nutritional guidance and counseling to patients and their families.
• Assists with changes to patient’s menu, including patient food preferences, and the development of standardized recipes, as well as assists in overseeing food service workers in proper food handing and portion control.
• Prepares, presents, and conducts individual and group education classes, as needed.
• Develops and utilizes effective lines of communication regarding nutrition information within the facility by coordinatingwith an interdisciplinary team on a daily basis to provide quality care to residents,
• Knowledge of applicable state and federal guidelines.
• Computer literacy and proficiency with EMR software and computer-based menu systems

Qualifications:

• Credentialed as a Registered Dietitian by the Commission on Dietetic Registration (CDR).
• Licensed by the NY State Dietetics Licensing Board.
• One Year of senior living experience- preferred.
• Possess the necessary skills to effectively utilize Microsoft office applications, electronic medical records, and nutrient analysis programs.
