Job summary

Sapphire Rehabilitation of Northtowns, located in Erie County, New York, is seeking a Registered Dietitian to join our skilled nursing team. The RD will assess nutritional status and recommend therapy aligned with patients’ needs, and assist dietary staff in delivering high-quality meals and a meaningful dining experience.

The role involves collaborating with medical staff to develop care plans, educating residents and families, and ensuring compliance with state and federal guidelines.

Qualifications

• Credentialed as a Registered Dietitian by the CDR.
• Licensed by the NYS Dietetics Licensing Board.
• One year of senior living experience preferred.
• Proficiency with MS Office, EMR and nutrient analysis programs.

Responsibilities

• Interviews patients and families for nutrition history and documents in the medical record.
• Performs nutritional assessments and develops patient-specific care plans.
• Recommends diet modifications and communicates with physicians for orders.
• Calculates nutrient content of tube feeding and dietary intakes as ordered.
• Provides counseling to patients and families on nutrition.
• Guides menu changes and assists in recipe development and portions.
• Delivers individual and group nutrition education.

Sapphire Rehabilitation of Northtownsan innovative skilled nursing facility, specializes in caring for the elderly is seeking a Registered Dietician to join our team of professional care givers Registered Dieticianwill be responsible for assessing the nutritional status of patients and recommending the appropriate nutrition therapy as it pertains to the patients’ nutritional needs. TheRegistered Dieticianwill also be responsible in assisting the dietary staff in creating high quality meals and the delivering of a meaningful dining experience for clients in a long tern skilled nursing facility.

Responsibilities:

• Interviews patient and/or family for nutritional history, obtains patient food preferences, assesses patients, and documents in patients’ medical record.
• Conducts nutritional assessment, develops and implements patient specific care plans, and assists in recommending nutrition care plans to medical staff for review and consideration.
• Assists in recommending therapeutic diet modifications and/or supplemental feedings to physicians for patients, and ensures accuracy of diet order received by checking against physician’s order.
• Calculates nutrient content of tube feeding and dietary intakes as ordered by physicians.
• Provides nutritional guidance and counseling to patients and their families.
• Assists with changes to patient’s menu, including patient food preferences, and the development of standardized recipes, as well as assists in overseeing food service workers in proper food handing and portion control.
• Prepares, presents, and conducts individual and group education classes, as needed.
• Develops and utilizes effective lines of communication regarding nutrition information within the facility by coordinatingwith an interdisciplinary team on a daily basis to provide quality care to residents,
• Knowledge of applicable state and federal guidelines.
• Computer literacy and proficiency with EMR software and computer-based menu systems

Qualifications:

• Credentialed as a Registered Dietitian by the Commission on Dietetic Registration (CDR).
• Licensed by the NY State Dietetics Licensing Board.
• One Year of senior living experience- preferred.
• Possess the necessary skills to effectively utilize Microsoft office applications, electronic medical records, and nutrient analysis programs.
